You've clearly done more reading than I have, but my gut is telling me it's a conglomeration of independently-acting interests with a shared goal of controlling human behavior & generating massive amounts of wealth. Going back to the Flexner Report...the robber barons saw the future value of monopolizing health care. We're still seeing the ripple effects of what Rockefeller & Carnegie did to medicine. I'll never forget reading "The Great Influenza" by John Barry during undergrad. Several times throughout the book he says something to the effect of "Rockefeller continued to see his homeopathic physician" (all while dismantling anything that didn't fit the pharma/surgery mold of John's Hopkins University).
